Legal reference & service status
This form follows the ordinary nonpayment framework in A.R.S. 33-1368(B), (G). Days are calendar days under G. Service and receipt require separate analysis under A.R.S. 33-1313. The PDF includes a blank delivery record; no actual delivery date or filing date is created here.
Arizona Courts notices · Pima forms
Omitting an address from this notice does not replace separate landlord name/address disclosure duties under A.R.S. 33-1322.
Phoenix, Glendale, Tempe and Mesa: Maricopa County. Tucson: Pima County. Court precinct depends on the actual address. Current municipal rental TPT is not automatically added; historic periods need separate review. This is a custom form, not an official court form.
Arizona 5-day notice: a practical landlord guide

When is this notice used in Arizona?
An Arizona 5-day notice to pay rent or quit addresses rent that was not paid when due. Under A.R.S. § 33-1368(B), the written notice identifies nonpayment and the landlord’s intention to terminate the rental agreement if rent is not paid within the notice period. Section 33-1368(G) defines these days as calendar days.
This is different from a notice about a health-and-safety violation, another lease breach or the end of a tenancy. Federal or subsidized housing, mobile-home and RV rentals, tribal land, lease terms and payment agreements can require a different review.
What should you have ready?
- The names on the lease and the full Arizona rental address.
- Each unpaid rental period, the rent charged and all payments or credits already received.
- Any reasonable late fee supported by the written lease, with its calculation.
- Clear payment instructions, your signature and the planned delivery method.
Check each rental period once. A previous balance should not be added again when its rent is already listed.
Delivery and the notice period
A.R.S. § 33-1313(B) describes hand delivery to the tenant and registered or certified mail to the tenant’s designated notice address or, if none, the last known residence. For registered or certified mail, deemed receipt is actual receipt or five days after mailing, whichever is earlier. Keep evidence of delivery or mailing.
The PDF includes a delivery record to complete after delivery. Creating, signing or downloading the notice does not deliver it. This tool does not calculate a court filing date; check receipt, applicable requirements and the court’s instructions before filing.
What if the tenant pays?
Before an eviction action is filed, § 33-1368(B) provides for reinstatement when the tenant tenders the past-due unpaid periodic rent and a reasonable late fee stated in a written rental agreement. Different amounts apply after filing. Review payments and any payment agreement before taking the next step.
A rent notice is not an eviction judgment. For court procedures and official alternatives, use the Arizona Judicial Branch’s eviction forms and notices. FriendlyLandlord’s document is a custom preparation tool, not an official court form or legal advice.
Questions about your notice and saved draft
Is the Arizona notice PDF free?
Yes. Complete the form, review your notice, draw or type your signature, then confirm and download. No account or payment is required to create the PDF.
Will correcting a field erase my signature?
Your drawn or typed signature stays available when you correct the form. Review the updated details and confirm again before downloading. A correction cancels the previous approval so the PDF reflects the notice you just reviewed.
Where are my form and signature saved?
When automatic saving is on, the draft stays in this browser profile on this device. It is not uploaded to FriendlyLandlord. Anyone using the same profile may be able to reopen it. Turn saving off on a shared device, and use Clear saved draft & form or Finish & clear draft when you are finished. Downloaded PDFs are kept.
Can I return to the form without internet?
First open the form online and wait for the message that it is ready to reopen offline. Check that your draft says it was saved. You can then keep working or reopen the same address without internet while the browser retains its saved data. Private browsing or clearing browser storage can remove it.
How do I check saving on my iPhone or Android?
Use Safari on iPhone or Chrome on Android in a regular tab. Try these steps with fictional details:
- Enter a few fields and draw a test signature. Check for the saved-draft message and offline readiness.
- Rotate the phone both ways. Check that your fields and signature remain and do not stretch.
- Switch to another app for 30 seconds, then return. Check the same details again.
- Turn on airplane mode and turn Wi-Fi off. Change one field, wait for the saved message, then refresh.
- Close the tab and reopen this exact page while offline. Check that the latest change and signature return.
- Reconnect, finish the fictional form, review and confirm, then download. Check the PDF’s details, signature and date.
- Repeat with Type signature. Finally clear the test draft and refresh; the form should stay blank.
After reopening or correcting the form, an unchecked review confirmation is expected. Your signature should still be present.
Something did not save? Keep the page open if it still shows your work. Report a form problem with your phone model, iOS or Android version, browser, the step that failed and the exact save message. Use fictional examples; do not send tenant information or a real signature.
