5. Review & download
Review, download and print. The landlord and delivery witness sign on paper.
Check required details
- The rental is inside Baton Rouge city limits. The written, fixed-term lease has already ended for unpaid rent under its termination clause, and every required payment warning, grace period and extra notice step is complete.
- This is a new notice for undisputed rent only, with no fees or other sums owed. There is no accepted payment for this default, pattern of accepting late rent, prior notice or case, payment plan, repair dispute, ownership change, sublease, special protection or other reason for ending the lease.
- I am the landlord signing this notice. I will print it, sign by hand and give each tenant a copy today with a witness. The five-day period starts from actual delivery; a download or share is not delivery.
If any condition does not fit, use “Different situation / not sure” to review it before printing.
Print and sign by hand before delivery. This PDF is not electronically signed.
Prepare the PDF, then download and print it. Share does not legally deliver this notice.
Check that your PDF was saved or shared successfully before clearing this draft.
The PDF uses the original court form. Landlord and witness signatures remain blank for signing by hand. No court case is filed.
Louisiana notice guide
This branch fills the Baton Rouge City Court general notice for a narrow residential nonpayment case. It includes a separate rent statement. It does not end a lease automatically or file an eviction case.
When this form fits
The rental must be inside Baton Rouge city limits. The written fixed-term lease must already have ended under its express nonpayment termination clause, after any required warning, grace period and other steps. If you are unsure whether the lease has ended, use a separate review before preparing this notice. A notice to vacate is different from a demand that gives the tenant a new opportunity to pay and stay.
Five days from actual delivery
The court asks for a relative five-day period, not a specific move-out date. Do not count the delivery day, weekends or Louisiana legal holidays. Do not use the court’s no-hearing dates as a holiday list. Check current orders before filing; this tool does not give a filing date. Keep a record of actual delivery.
Print and hand-sign
Give each tenant a printed, hand-signed copy on the notice date, with a witness. Keep a copy signed by the landlord and witness. Witness details and signatures are completed on paper. This branch does not support mailing, posting or email service. Louisiana’s electronic-transactions law excludes primary-residence default and eviction notices; no separate electronic-signature basis has been verified here. Download or Share only handles the file.
Different situation?
Get a separate review for housing assistance or covered/unknown loans; fees, payments or payment plans; previous notices, accepted late rent or disputes; repairs, domestic abuse, stalking, military or other special protections; periodic, expired or oral leases; subleases, ownership changes, tribal or disputed jurisdiction; or any lease requiring more time or steps. A payment or new event after preparation needs review before further action. This notice does not authorize lockouts, utility shutoffs or removing anyone.

Phone and saved-draft help
Fill the form on your device and download the PDF. Print and sign it by hand before giving it to each tenant today. Saving stays in this browser; keep the page open if saving is unavailable. Download or Share does not establish legal delivery. Physical iPhone/Android and real device sharing have not been verified.
Something did not save? Keep the page open if it still shows your work. Report a form problem with your phone model, iOS or Android version, browser, the step that failed and the exact save message. Use fictional examples; do not send tenant information or a real signature.
